Massage Therapy Eligibility Process:
-
Referral for Massage: You’ll need a referral from a primary care provider (e.g., doctor, chiropractor, physical therapist) to initiate SGI- or WCB-approved massage therapy. Marieta Miedema can provide you with a Referral Form for your provider to fill out.
-
SGI- or WCB-approved Massage Therapist: Marieta Miedema is an SGI- & WCB-approved care provider. To find other SGI- or WCB-approved Registered Massage Therapists (RMTs), go to the Massage Therapist Association of Saskatchewan website: www.saskmassagetherapy.com.
-
Claim Information: Provide your chosen approved RMT with your massage referral form, claim number, date of birth, adjuster's name, and phone number.
-
Authorization Form: Your RMT will submit a Request Authorization to Treat form to your adjuster for approval before your massage treatments can begin.
-
First Massage: Your RMT will email you a Health Intake Form which you can easily fill out & submit using your smartphone or PC BEFORE your first appointment. Your first RMT appointment will consist of a 30min assessment & 30min treatment.
